A)	Solvency Certificate
	a) Form a Revenue Officer not below a Tehsildar. Incase of solvency from outside the state of M.P.It be countersigned by Collector or Dy.Collector.
	b) Date of granting the solvency cerificate by the officer (it should be current its period of validity is required.
	c) Amount.
	d) In the case of limited concerns the last audited balance sheet to be furnished.

B)	FDR/Deposit

A list showing the name of works executed by him in the Past with cost and year of completion.

Profession followed so far year wise experience certificate for last 5 years from officers not below the rank of E.E

Experience in Contract works with P.W.D or other Department.

Attested copies of certificate from officials giving name and designation of officers (as per col.No.10 above)

a)	Whether there are technically qualified persons working with the applicant with details.
b)	If not whether the applicant is ready to appoint an overseer/Engineer qualified from an Institution recognized by Govt. of M.P.

Amount of income assessed and Tax paid for last three years. Current income tax clearance Certificate to be attached.

And other details which the applicant considers materials for the purpose. List of plant & equipment to be attached.

In case of firm, the Name of each Partner should be given with attested copy of a partnership deed along with the acknowledgements obtained from the register to firms.

Name of persons authorized to sign on behalf of contractors (receipt of bills etc) should be specified copy of General power of attorney to be attached.

18.

In the contractor is interested in Electrical works only, he should be submit a copy of license issued by Electricity Department that he has in his employee a wireman holding license.
